Job Description
This position serves as the quality management and/or education consultant for case management and continuum of care throughout the healthcare system. Participates in the development of department/system plans for case management and continuum of care. Responsible for the systematic process of identification, analysis, planning/development, recommendation, communication and education to ensure standardized best practices are implemented in regions, facilities, and defined service areas across the continuum. Collaborates with stakeholders -Case Management, Continuum of Care, Patient Flow, Compliance, Physicians, Clinical Operations, Clinical Education, Region Education Directors, Allied Health Services, and Revenue Cycle Organization leaders- to define quality measures and improve case management/continuum of care outcomes. This position is responsible for monitoring and improving evidenced-based practice by demonstrating expertise in case management practice, writing, education, communication and group facilitation.
Minimum Requirements
Current license to practice as a registered nurse in the state of Utah.
Bachelor's degree in Nursing. Degree must be obtained through an accredited institution. Education is verified.
Minimum 2 years hospital case management, care management, care coordination, patient flow, utilization review, discharge planning, or similar experience in an acute care setting.
Experience in a role requiring strong communication and analytical skills, and expertise in data management,
Experience in a role requiring work in a fast-paced and changing environment while handling multiple deadlines and priorities.
Experience using word processing, spreadsheet, database, internet and e-mail and scheduling applications
Preferred Qualifications
Master's degree in Nursing or a healthcare-related field.
Demonstrated expertise as a case management educator.
National certified in case management (i.e. ACM, CCM, other equivalent national certification).
Healthcare education certification. Experience in medical writing.
ICQI InterQual Certified Instructor (or eligible to certify at time of hire)
Project mangement/program development exp.
